A notice, substantially in the form attached to the Plan as Annex A, by which a Director advises the Company to defer the payment to such Director of all or a portion of the Director Fee due or to become due to such Director (specifying the dollar amount of Director Fees to be deferred (or in lieu of a specific dollar amount, the percentage of Director Fees that would be due from time to time, to be deferred)
