Election Form

Election Form. The form as the Committee may designate from time to time which shall be used for deferring payment of Fees in accordance with the provisions of the Plan and shall also include any prior forms used in connection with the Plan for the purpose of deferring payment of Fees

Election Form. The form which an Eligible Employee or Eligible Trust Manager shall be required to properly complete in writing and timely file at least 15 days prior to the commencement of any Purchase Period in order to make any of the elections available to an Eligible Employee or Eligible Trust Manager under this Plan

Election Form. The form specified by the Program Administrator on which a Participant makes a Deferral Election.

Election Form. The form provided by the Company pursuant to which Executive specifies the date on which benefits will commence and the form of the benefit. Executive must execute the Election Form within 30 days following the Effective Date of this Agreement

Election Form. The form which an Eligible Employee shall be required to properly complete in writing and timely file in order to make any of the elections available to an Eligible Employee under this Plan.

Election Form. With respect to each Participant, the form specified by the Plan Administrator (which may include an electronic form), as completed and delivered to the Company by each Participant pursuant to such deadlines as may be determined by the Plan Administrator

Election Form. The form acceptable to the Committee which an Employee shall use to make an election to purchase Shares through Payroll Deductions pursuant to the Plan or to decrease or discontinue Payroll Deductions during an Offering pursuant to Section 5(b) below.

Election Form. The meaning set forth in Sections 3(b) and (c).

Election Form. The form an Eligible Director or Eligible Employee must complete and timely file with the Plan Administrator to make any of the elections available to Eligible Directors or Eligible Employees under the Plan.

Election Form. The Election Form for Special Equity Award in substantially the form attached hereto as Exhibit B.
