Grant Notice

Grant Notice. The Notice of Restricted Share Unit Award to which these Award Terms are attached as Exhibit A

Grant Notice. The Notice of Stock Option Award to which these Award Terms are attached as Exhibit A

Grant Notice. Means the Notice of Stock Option Grant accompanying this Agreement pursuant to which Option Holder has been informed of the basic terms of the Option evidenced by this Agreement.

Grant Notice. The notice provided to a Grantee that he or she has been granted an Award under the Plan and which includes the name of the Grantee, the type of Award, the date of grant of the Award, number of Ordinary Shares subject to the Award or potential cash payment right, (if any), the vesting schedule for the Award (if any) and other key terms applicable to the Award

Grant Notice. The Notice of Grant of Stock Option accompanying the Agreement, pursuant to which Optionee has been informed of the basic terms of the Option evidenced hereby

Grant Notice. Shall mean a written notice evidencing certain terms and conditions of an individual Option grant. The Grant Notice is part of the Option Agreement.

Grant Notice. The Restricted Stock Unit Grant Notice (the "Grant Notice") to which these Terms and Conditions are attached (together with the Grant Notice, this "Agreement"), AFFYMETRIX, INC., a Delaware corporation (the "Company"), has granted to Recipient the right to receive the number of Restricted Stock Units (the "RSUs") under the Plan as set forth in the Grant Notice (terms used but not defined herein have the meaning set forth in the Grant Notice or the Plan).

Grant Notice. The Stock Option Grant Notice (the "Grant Notice") to which these Terms and Conditions are attached (together with the Grant Notice, this "Agreement").

Grant Notice. Means the Notice of Restricted Stock Award accompanying these Award Terms.

Grant Notice. The Notice of Performance Share Award to which these Award Terms are attached as Exhibit A
