Patient Records

Patient Records. Shall include, without limitation, any and all medical charts, files, notes, transcripts, x-ray files, lab reports, other diagnostic information or materials, insurance information, billing and payment statements or records of any kind, explanations of benefits, and other information of or relating to any patient of the Hammond ASC, Group or Providers, as the case may be, of any kind and in any form whatsoever.
